What is Project 4031?

Project 4031 is a non-profit organization that seeks to improve end-of-life stories for patients and their families by providing funding for basic needs and fulfilling final dreams.

Where did the name Project 4031 come from?

Project 4031 was named for the Old Testament verse, Isaiah 40:31. “Those who hope in the Lord will renew their strength. They will soar on wings like eagles. They will run and not grow weary. They will walk and not grow faint.”

Who does Project 4031 help?

Project 4031 assists people of all ages who live in North Texas (living in or primarily served by a hospital or hospice in the following counties: Collin, Cooke, Dallas, Denton, Ellis, Erath, Fannin, Grayson, Hood, Hunt, Johnson, Kaufman, Montague, Navarro, Palo Pinto, Parker, Rockwall, Somervell, Tarrant and Wise) and that are considered hospice eligible according to Medicare standards. This means that a medical professional will attest that the patient has a life expectancy of six months or less.

How does Project 4031 help?

Project 4031 has two programs to assist people at the end of life and their families: Funding for Families and Fulfilling Dreams. Through Funding for Families, Project 4031 provides financial support for basic needs such as food, utilities, and rent. Through Fulfilling Dreams, Project 4031 helps to grant a final wish.

What is Project 4031’s funding limit?

As a small organization and due to the tremendous need of the demographic we serve, Project 4031 is limited in what we can spend on each applicant. Support is determined on a case-by-case basis and the number of applications received. In 2021, the average amount spent per patient was $755. Services are provided on a temporary basis, usually for 1-2 months depending on the nature and cost of the request.

Is there an age limit for dream requests?

No. Project 4031 is proud to grant dreams to people of all ages which differentiates us from other wish granting organizations.

Is there an income limit for assistance?

No. While you will be asked to submit proof of income, your income level does not determine whether assistance is provided.

How can I apply?

Applications for Funding for Families and Fulfilling Dreams are available on our website by clicking on the “Apply Now” button at the top of the home page.

My loved one is dying and needs help now. How long does it take to get assistance?
From the time a completed Funding for Families application is submitted, approved bills are usually paid within 48 hours. For Fulfilling Dreams, once a completed application is submitted, you will be contacted withing 24 hours to discuss ways to make the dream a reality.
Is the application process lengthy?

Because we are meeting people at the end of life, Project 4031 seeks to make the application process as simple as possible. A completed application must be submitted to receive services which includes required proof of income documentation and current copies of any bills that are being requested for payment. In addition, a hospice representative or medical professional must complete a medical authorization stating that the applicant is eligible for assistance.
